 Note: Those numbers that are followed by an asterisk (*) are reverse-scoring items. For those items‚ a score of 1 should be entered as 5‚ 2 becomes 4‚ 3 remains 3‚ 4 becomes 2‚ and 5 becomes 1. Be sure to reverse these items as noted.

 The higher the score in each category and overall‚ the higher the level of plateauing. This survey can be used to assess the need for resiliency building in any of the three plateauing categories or regarding overall plateauing.
